I was curious as to why we do not see the goose/swan neck wrist lock in the famous film of Otsuka performing Kumite Gata with his son
In KG #2 at 53 seconds. The wristlock is clearly absent. When and why was the lock added and who added it?

I can't answer for the instance in the filming but I know in some of the practices we do, it depends on the moment. You may choose to apply a downward and backward kuzushi, an upward gooseneck pin if their arm is appropriate. In that footage he turns the arm like in the first idori assuming the first one is the same for all of us :-) eg the double handed wrist pin.

That brings me on to my next question. Being kata are the moves not set as opposed to being more pragmatic (if thats the right word). If you were performing this at a grading would you be marked down for not doing the lock?

I don't think that would be a case for marking down although I have never been a grading examiner or even asked the question to one. On kihon kumite 1, is the last strike above or below the arm of ukemi? Don't think it means much if the whole package doesnt look right as a strike to the throat could be done and I don't think the examiners I have faced would make too much of it if overall everything is in line.
You may then ask where do we draw the line. I reckon if you did uraken jodan instead then you might get pulled up for taking the mick, but then again if the attacker was messing up and you had to improvise then I bet that would be OK as well. I trust the examiners.

Is it this one?

If so, it's called kamate-kime (鎌手極め) or "sickle hand lock". In the UK, this is called a "gooseneck" in the army and police.

In both cases the beginning is different but I believe the kuzushis' main points are the same as in the one that shep probably means.
I could speculate that the reason for the different ways of finishing what is many times called Kumite gata 2, is that both are natural endings and also related in such a way that if you feel one isn't working you can still try the other one or take it further.
