Now when I hold both arms straight in front and then raise them I get to about 45 degrees with my right arm and pain kicks in.

Actually having written this, I did a couple more and managed to clear the pain and it can move more freely now.

Hmm sounds like the onset of frozen shoulder. Is it starting to give you pain at night? Some people describe it as toothache of the arm. If it is, the good news is that it eventually goes away, won't tell you what the bad news is.

Look in to Myofacial Trigger Points.

Not necessarily pain at night but I can feel it niggling. I just thought it was because of sleeping on my right side for years.

I am little dubious about some of the people listed here about their frozen shoulder treatment. George Michael, Borat and Bruno, who knew.

So, I am assuming that you are coming round to the idea that it might be frozen shoulder?

I haven't seen a doctor yet so have no other answers.
