Now when I hold both arms straight in front and then raise them I get to about 45 degrees with my right arm and pain kicks in.
Actually having written this, I did a couple more and managed to clear the pain and it can move more freely now.

Hmm sounds like the onset of frozen shoulder. Is it starting to give you pain at night? Some people describe it as toothache of the arm. Not necessarily pain at night but I can feel it niggling. I just thought it was because of sleeping on my right side for years.
